Safety First - Working Together Agreement and Assumption of Risk

 

We are very excited to welcome you to Studio4 All Dance . We have been working hard to make your experience as safe as possible. Please read thoroughly and sign.

 

I (or signing on behalf of child under 18) hereby agree to the following:

• I acknowledge that my child/children's participation in aerial workshops entails known and unanticipated risks including, among other things: slips and falls; falling from equipment; rope burns; pinches, scrapes, twists and jolts that could result in scratches, bruises, sprains, lacerations, fractures, concussions, or even more severe life threatening hazards; strains, cuts, bruises, muscle soreness, and fractures; musculoskeletal injuries including head, neck and back; injuries to internal organs up to and including paralysis or even death.

 

• Studio4 All Dance will always teach in a safe manner with respect of each other and equipment. Should anyone demonstrate behaviour that does not reciprocate our teaching ethos we will dismiss the person(s) for safety of themselves, the instructors, and other students.

 

• I understand that circus skills include highly physical activities that may cause injuries, and often require instructors to assist students as part of the learning and spotting process. I understand that all instructors at Studio4 All Dance are highly qualified and will use their professional discretion when assisting each student

 

• Any injuries or accidents that happen within the class, Studio4 All Dance cannot be held responsible.

 

What we ask of you:

 

• To be safe: understanding that aerial can be dangerous especially when not listening or becoming distracted. If we feel someone is being unsafe we will give warnings, and finally ask you to leave the class for safety reasons if we feel necessary. We will cover rules at beginning of classes, courses & terms and continuously if needed, so each student is aware of ‘how to be safe in the air’

• All students must wash their hands thoroughly before activity begins.

• To be respectful of each other and to listen to each other.

• Any prior injuries must be discussed before session.

• All jewellery must be removed, long hair tied back, and comfortable clothing worn i.e.: jogging bottoms/leggings.

Photography and filming consent:

During the class photos and filming may take place. This can be used for business advertisement only. This might include (but is not limited to), the right to use them for online publicity, social media, press releases. If you prefer your child not to be included please do make us aware.
